Practice Policy

  • We need 3-day notice for prescription refills.

  • We need 2-day notice for appointment cancellation or reschedule.

    • $50 fee will be applied if notice is not given.

    • We are booked for months, thus not showing up for an appointment will take away the opportunity of another patient to be seen sooner.

  • Prior authorizations can take up to one week to complete.

  • If you are 15 minutes late, the appointment will need to be rescheduled.

  • We hate missing phone calls. Please leave a voicemail. We will return your phone call in a timely manner.

  • For non-emergency issues, we prefer the use of patient portal as it facilitates the patient-physician communication in a clear, accurate and timely fashion. Patient portal will allow you to have direct access to Dr. Tashko.

  • In case of an EMERGENCY, do not send a patient portal message, instead, immediately call 911 or reach us via the on-call service at 301-658-3586 #2.

  • For new patients:

    • Please fill out the above forms prior to your appointment.

    • Arrive 15-30 min prior to your appointment.

    • Bring your ID and health-insurance card.

    • Bring with you the complete list of:

      • All the doctors you see, even intermittently. We need to be in contact with your doctors to coordinate, streamline and facilitate your care.

      • Medical illnesses

      • Surgeries

      • Medications

      • Drug Allergies

      • Illnesses that run in the family

      • Pharmacy Information

Extra information on how to fill out the forms prior to your appointment:

  • You can download the forms, print and fill them out, or

  • You can download the forms and fill them out electronically by using the free software/app โ€œAdobe Fill and Signโ€.

    • To have free access to the application, you need to create an Adobe login and password first by downloading the โ€œAdobe Acrobat Readerโ€.

    • Once โ€œAdobe Acrobat Readerโ€ is downloaded and you have logged in, the โ€œFill and Signโ€ feature is found on the right side of the application.
